Donation Receipt Request (Bank Transfer)

If you donated via bank transfer, a donation receipt can be issued after your deposit is confirmed. Please provide the exact depositor name, date of deposit, and donation amount. Thank you.

Important Notice: If you are a regular donor or donated via credit card or a simple payment method, your donation details are automatically registered. You do not need to fill out this form

    Consent to Collection and Use of Personal Information *

    1. Items collected and used: name, resident registration number, contact number, address, date of deposit, donation amount
    2. Purpose: issuance of donation receipts
    3. Retention period: 5 years from issuance date under the Corporate Tax Act Article 112-2 obligations
    4. Third-party provision:
      – Recipient: National Tax Service (NTS)
      – Purpose: tax credit processing and verification of donation receipt details
      – Items: name, resident registration number, donation amount
      – Retention by recipient: 10 years
    5. Processing entrustment:
      – Scope: donor database management

    Note: If you do not agree, a donation receipt cannot be issued.
